What options do I have for my wide, shapeless nose? (photos)

Dear Hanna:    Rhinoplasty (nose reshaping surgery) to address a wide nose is a common request. Most likely, an “open rhinoplasty” (where the soft tissues are gently elevated to allow direct access to the tip cartilages) will be required. These tip cartilages are then sculpted and sutured to refine the nasal tip. Osteotomies are required to reposition a wide nose closer to the midline.If you like, please follow the link below.  A closed rhinoplasty approach can accomplish narrowing a wide nose by making all of the incisions on the inside of the nose. To narrow wide nasal bones requires osteotomies. A tip rhinoplasty with suture techniques and  a conservative amount cartilage removal are required to narrow the wide tip. Wide nostrils are narrowed with an alar-plasty.  For many examples and more information, please see the link and the video below

While a single frontal view picture is a limited assessment of your nose you do appear to be a candidate for rhinoplasty. You do appear to have thick/ fleshy nasal skin. It will be very important for your chosen surgeon to explain to you exactly what is possible and what you can expect from typical rhinoplasty results. If the appearance of your nose concerns you, it can be
improved with an artistically performed rhinoplasty. What would be the best
option and exact procedure would depend on an examination and discussion about
desired outcome.

Keep in mind that following advice from a
surgeon on this or any other website who proposes to tell you exactly what to
do based on two dimensional photos without examining you, physically feeling
your tissue, assessing your desired outcome, taking a full medical history and
discussing the pros and cons of the operative procedure would not be in your
best interest. Natural appearing results need to be individualized and what is
appropriate for one patient is not necessarily the best for someone else. I
would suggest that you find a surgeon with extensive rhinoplasty experience
certified by the American Board of Plastic Surgery and one who is ideally a
member of The American Society for Aesthetic Plastic Surgery (ASAPS) or a
facial plastic surgeon(otolaryngologist) that you trust and are comfortable
with. You should discuss your concerns with that surgeon in person.

A rhinoplastic surgeon has many tools at his/her disposal to deal with wide shapless noses.Among them are osteotomies, appropriate reduction if alar reduction and cartilage grafting to the tip. Without a proper examination, it is not possible to say anything specific, other than, very thick skin usually does not let you get a delicate looking nose. But a significant improvement is possible.
