What exactly is a pocket modification? I'm looking for a doctor in Arizona who does this.

A pocket modification is a fairly routine occurrence that we do  when the implants have moved out of their usual position. Usually they migrate downward and outward such that the breast look great in a bra but to low when standing naked and toward the armpits when laying on ones back. The most common culprit is weight lifting and/or forceful contractions of the chest (pectoralis) muscles. I am sorry to hear about your concerns after breast augmentation surgery. Exactly what type of "pocket modification" is necessary in your case will depend on your physical examination,  concerns, and goals.   My best advice: select your plastic surgeon carefully. Make sure that he/she has significant experience achieving the types of outcomes you will be pleased with. Then, communicate your goals carefully, as discussed above. From your description the pocket modification you are describing would be a capsulorrhaphy.  This is an internal suturing technique used to close off a portion of the pocket to reposition an implant.  It commonly occurs in patients who went aggressively large relative to their own natural breast tissue and size and I usually recommend patients consider a reduction in implant size at the same time.  Obviously this would require bilateral surgery but may be in your best interest long-term.  Discuss this option with your selected ABPS board-certified plastic surgeon.
