What exactly is a pocket modification? I'm looking for a doctor in Arizona who does this. I have 600cc silicone implants, under the muscle. Right breast is hanging lower, one year PO. Been told all I need is a pocket modification. One doctor quoting $2400. Anyone specialize in this!? I'll make an appointment!

A pocket modification is a fairly routine occurrence that we do  when the implants have moved out of their usual position. Usually they migrate downward and outward such that the breast look great in a bra but to low when standing naked and toward the armpits when laying on ones back. The most common culprit is weight lifting and/or forceful contractions of the chest (pectoralis) muscles. The internal repair is done with permanent suture and the malposition happens with both large and small implants.However, the repair is beautiful, predictable, and the patient can change implants if desired..

First you need to find a surgeon that knows how to deal with complex breast problems.  Pocket modification with removal and replacement of implants should cost around $9,000 to $10,000.  So anyone who is charging you $2400 you should be very wary of.  They are not doing the right operation, they are not qualified, or they are not experienced.  You get what you pay for.

Great question.  A pocket modification for breast implants is any surgical change to the breast implant pocket to help achieve a better breast shape and appearance.  It sounds like the type of pocket modification that you would be having is a capsulloraphy.  A capsulloraphy is the process of suturing the breast implant pocket so that the pocket and implant sits higher up on the chest.  The fee for this will vary depending on the amount of work needed, whether its one side or both, and from surgeon to surgeon.Good luck with your breast revision surgery.Dr. Remus Repta

I am sorry to hear about your concerns after breast augmentation surgery. Exactly what type of "pocket modification" is necessary in your case will depend on your physical examination,  concerns, and goals.   My best advice: select your plastic surgeon carefully. Make sure that he/she has significant experience achieving the types of outcomes you will be pleased with. Then, communicate your goals carefully, as discussed above. From your description the pocket modification you are describing would be a capsulorrhaphy.  This is an internal suturing technique used to close off a portion of the pocket to reposition an implant.  It commonly occurs in patients who went aggressively large relative to their own natural breast tissue and size and I usually recommend patients consider a reduction in implant size at the same time.  Obviously this would require bilateral surgery but may be in your best interest long-term.  Discuss this option with your selected ABPS board-certified plastic surgeon.
