Breast implant after reduction for upper pole without creating massive breasts? (Photo)

Question

I had a breast reduction in 2018, at the time I was 180lbs and 5’4, 1400 grams were removed. I have since lost 40 lbs (5’4 and 140) and although my breast didn’t shrink I’d like them to have more upper pole fullness. Unfortunately even with the reduction and 1400 grams removed I still have fairly large breast. Is it possible to add an implant for upper pole but not create massive breasts. Is this very complicated due to my existing scars?

This is possible and happens somewhat frequently. It is fine to use a modest size implant to get the upper fullness without making a huge breast. Often in this procedure, it is necessary to redo the incision under the breast to get the best shape. This allows access to place the implant.

Hello @toomuchteetee thank you for your question and showing your pictures. adding a prostheses for upper pole fullness will make the breast heavier and sagging. If you are going to have breast surgery, I suggest a lift. For the most adequate aesthetic result over time what we want is to raise it and rearrange it, to put it in its place, so that the projection of the breast from the anterior axillary line to the external midline and give it projection. Your result overall is good. I would suggest a modest size implant around 359 cc's. At the same time the lower portion of the breast skin can be tightened where it stretched out.

breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.

This is a great question. The structure of the implant will allow for improved upper pole fullness and projection of the breasts after a reduction or a lift. I would recommend a smaller implant 250-350 cc that is a moderate to moderate plus profile. The implant can be placed through existing scars.

Hello. Of course breast implants are not only to add volume, but also to fill upper or lower poles. Your condition is consider a "pseudoptosis" meaning that the upper part or superior pole of the breast is empty. Implants will be use not to increment volume but to fill the upper pole.

Hello and greetings from Turkey. You can have breast implant operation with a small implant. The main goal of performing breast implant operation is creating the upper pole not the enhance the size of your breast. The procedure is not complicated and I did perform many of the procedure like that during my practice. Since you had a procedure before, I strongly suggested you to contact with a board certified surgeon who is experienced in breast procedures.
