I had a breast reduction in 2018, at the time I was 180lbs and 5’4, 1400 grams were removed. I have since lost 40 lbs (5’4 and 140) and although my breast didn’t shrink I’d like them to have more upper pole fullness. Unfortunately even with the reduction and 1400 grams removed I still have fairly large breast. Is it possible to add an implant for upper pole but not create massive breasts. Is this very complicated due to my existing scars?

It is fine to use a modest size implant to get the upper fullness without making a huge breast. Often in this procedure, it is necessary to redo the incision under the breast to get the best shape. This allows access to place the implant.

Hello @toomuchteetee thank you for your question and showing your pictures. adding a prostheses for upper pole fullness will make the breast heavier and sagging. If you are going to have breast surgery, I suggest a lift. For the most adequate aesthetic result over time what we want is to raise it and rearrange it, to put it in its place, so that the projection of the breast from the anterior axillary line to the external midline and give it projection. The best course of action is to consult one or several board certified plastic surgeon for more information and options tailored specifically to your needs. Best wishes! Alan Gonzalez MD FACS

Answer: Breast reduction Your result overall is good. I would suggest a modest size implant around 359 cc's. At the same time the lower portion of the breast skin can be tightened where it stretched out.
