Drooping or squaring off of the lower inner part of the buttock typically comes with the aging process giving way to gravity and/or genetic variation. Either way this “drooping” of skin and fat in this area is usually not well corrected by shear volume augmentation, as you have realized from round 1 BBL, but usually requires lifting of this area with a lower buttock tuck. In doing so it also aesthetically shortens and/or rounds out the buttock cheek. It is ideal for those patients that have had significant weight loss or fluctuations, changes from fillers injected, multiple pregnancies, or who’s derrieres have succumb to the dreaded effects of gravity and aging. If the waist was smaller at one time, the waist can likely be reduced to those same measurements or smaller provided the muscular and fascial integrity has remained the same. From the one photo that you posted, it appears you could get an improved shape with another BBL. The area that you pointed out is actually very common and occurs frequently as we age. This means it gets worse over time as we get older and things sag more. This can be improved with another BBL, but will worsen over time.
High definition VASER liposuction can be used to correct this deformity. Fat transfer using this technique is much more efficient in that more fat can be harvested and the fat is more viable. The area of concern is not treated with a BBL and is not caused by a BBL. Some people refer to that as the dirty diaper look(not very nice). You can be corrected with a skin removal procedure. dr. Mendieta in Miami has described this with a procedure he calls the Miami thong lift. I think he has a few YouTube videos on the subject.You’re not a particularly great candidate for liposuction or fat transfer because of the skin laxity.
