About a year and two months ago I got my primary done. I had asked for a upturned and a harmonious / natural nose to make my face more feminine. I waited a year before making any judgement on my nose but after month 6 it had genuinely stayed the same since. I’m concerned my results might not align with what i initially envisioned. I notice my nose tip is larger than pre op and my side profile looks awkward and masculine. My facial profile now from the side is quite flat and my nose sticks out quite a bit. Will a revision rhinoplasty help improve my facial harmony or is it a deeper issue with my bone structure ? What is making my face look so tired and old? I would love to hear your opinion and thoughts thank you.

At fourteen months, it is reasonable to seek a revision-rhinoplasty consultation because most healing has occurred. In the photograph, the nose appears somewhat projected and the tip may not have the rotation or refinement you expected, but this single oblique view cannot show the frontal width, base, symmetry, airway, or the relationship of the nose to the chin and midface. I would not conclude from it that your nose makes you look tired or old.

Revision may be able to adjust tip projection or rotation and refine the dorsal profile, but it should be planned around the entire face rather than an isolated request for a smaller or more upturned tip. A flatter facial profile can also reflect chin and midface anatomy; that does not automatically mean those areas need surgery, but they should be evaluated before changing the nose again.

Revision surgery is more complex because of scar tissue and potentially limited cartilage. Bring your preoperative photographs, operative report, and any imaging or airway history. Obtain standardized frontal, profile, base, and smiling views, and compare at least one independent revision opinion with your original surgeon's assessment. The goal should be a small, specific improvement with realistic limits, not correction of a vaguely defined sense that the whole face is out of harmony.
