When doing an explant surgery, it is tough to visualize the final result. My best advice is to ask your surgeon to show photos of patients with clinical features similar to your body and implant characteristics. It is a complex situation; removing the implant will not put your body back when you decide to place the implants. Other factors that affect the result are the implant size, the plane of implant placement, and the present quality of the tissues.

However, is important that you have total capsulectomy. It is important to note that after the removal of implants, the skin and tissues of the breast may be stretched or sagging, which can lead to a less-than-ideal appearance.

If you do decide to have a lift, the most optimal and aesthetically adequate option would be immediate reconstruction with the BRALAN technique (Breast Reconstruction Anatomical Lift Augmentation) combine with a breast lift to provide a natural and lifted appearance. If you remove your implants and do not replace them, you will lose the fullness in the upper portion of your breast. Your nipple position appears appropriate and would likely not require a lift for repositioning; however, a breast lift would help tighten any excess skin following explant. You may also want to explore options such as autoaugmentation with fat transfer following explant to help maintain some volume. Ultimately, you have a variety of options to discuss with your plastic surgeon.

Your photograph shows that your nipple and areolar complex sits well above your inframammary fold so it will likely remain there after explantation. It's hard to predict and one problem with explants is deflation and extra skin. In my opinion you don't need a lift and this would likely result in a flat chest with breast lift scars. I'd suggest doing an explant with fat transfer to replace some of the implant volume with a fat transfer.

The technique I recommend is explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired.

You will look deflated. You might consider adding minimally invasive radiofrequency skin tightening like Renuvion to your removal.

After fifteen years your skin has stretched to accommodate the implants. When they are removed your skin will retract to a degree but some loose skin is likely to remain and the breasts may look "empty". A lift can be used to shape the breast and remove excess skin.
