I am looking to have my textured implants removed alongside the capsules. I am contemplating doing a lift either with my explant or as a secondary surgery. If I explant only & have pocket collapse or nipple inversion afterwards, what would fix this? Would a secondary surgery of a full anchor lift be curative of pocket collapse and nipple inversion following explant?
Answer: Inverted nipple & pocket collapse Hello, thank you for your question. It sounds like you are considering significant changes to your breast structure with explantation of your textured implants. Please see the attached video that answers your question specifically in detail. Choosing to remove textured implants and considering a lift is a significant decision. If you decide to proceed with an explant only, be mindful of the potential for changes in breast appearance. If pocket collapse or nipple inversion occurs, a full anchor lift can be a corrective solution. Thoroughly discuss all of your options, concerns, and desired outcomes with your plastic surgeon to develop a tailored approach that suits your needs.

Answer: What would fix an inverted nipple and pocket collapse after explant? Hi @NikkiR, thanks for your question. My opinion is that breast implant removal should be combined with immediate breast augmentation to reposition the breast tissue. However, it is important that you have a total capsulectomy. It is important to note that after implant removal, the skin and tissues of the breast may be stretched or sagging, which can result in a less-than-ideal appearance. If you decide to undergo a lift, the most optimal and aesthetically appropriate option would be immediate reconstruction with the BRALAN technique (Breast Reconstruction Anatomical Lift Augmentation) combined with a breast lift to provide a natural and lifted appearance. I link you to a video that sheds more light on the subject. If you are considering breast augmentation, consultation with a plastic surgeon experienced in this procedure is essential to ensure optimal results. Best regards. Alan Gonzalez MD, FACS. Answer: Considering explant A lift removes excess skin and reshapes the breast. This can often be performed at the time of explant. Most patients who undergo implant removal alone do so without significant deformities. It is important to note that if you have larger implants, current skin laxity, and/or the implants have been in for several years that the skin envelope will be permanently stretched. This is why many patients opt to have the lift at the same time.

Answer: Explant The pocket will collapse, and the nipple may or may not evert once the implant is removed. A lift may help correct the pocket. If needed, fat can be transferred to add more volume. However, fat is not placed in the area of the implant, so it does not expand the pocket. Your breasts will not look like they did prior to surgery because of stretching all the tissue.
