Interested in breast implant removal - is it necessary? (Photo)

Textured implants have been associated with BIA-ALCL. You have some fluid present but is not significant. However, removal could be done. The question is whether you want removal only or removal with placement a different type of implant or removal and a breast lift? If you undergo removal only, you will be dissatisfied with the hanging skin. When I remove implant I also do a simultaneous breast lift using The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery.

However, given the history of your Allergan textured implants and the potential risks for BIA-ALCL, it's essential to stay vigilant and maintain communication with your healthcare provider. The risk of ALCL is exceedingly rare. With textured implants and some fluid around the implants it would be a good idea to replace them with a smooth round implant and also remove the capsule.

Hello and thank you for your question! It is always best to be evaluated by a board-certified plastic surgeon in person.
Common reasons for re-operation include if patients are having difficulty with pain, masses, capsular contracture, fluid collections, etc or if patients would like to change the cosmetic outcome of their breasts. If patients are not having any problems then there is not necessarily a need to operate. The FDA does not recommend removal of textured Allergan implants without symptoms of BIA-ALCL.

If you are interested in having your implants removed you can do so at any time. I encourage you to visit with the provider who ordered the MRI or your plastic surgeon to discuss the results and your concerns. The risk of developing BIA-ALCL is very low, but it is best to address your concerns of necessity beyond a "yes" or "no" response.

With textured implants, a small amount of fluid may be present. If you are happy with your implants, then do a yearly ultrasound or mammogram just to check the implants. Ultrasound has no X-ray exposure. Your MRI appears to be normal, and shows no reason to remove the implants at this time. Remember, that tumor is rare.

I suggest you follow up with your plastic surgeon if they are still available to discuss your current implants.
If your current provider is not available, then I suggested scheduling a few in person consultations with other plastic surgeons in your community.
