Do I need to do breast lift revision with implants in separate procedure from tummy tuck? (Photos)

Question

I want to get a breast augmentation and a tummy tuck at the same time. I have previously had a breast lift which has since bottomed out and the areolas sit a bit too high. Been to a consultation where the surgeon wants to do two surgeries. First revision breast lift with implants and then remove skin from the underside of my breast to minimize the elongated look of my breast. Then he also suggested doing implants with a mini profile as that will minimize the chance of the implant being too heavy and bottoming out again. The reason he does not want to do the tummy tuck at the same time is because that could cause tension and that the breast lift scar under the breast will be dragged down. Especially since he prefers doing breast first before moving onto the tummy tuck in one surgery. While other surgeons tend to do the tummy tuck first and then breast. So my question is should I do it in two sessions or is it possible to have an okey result doing both at once?

Thank you for your question and photos. Based on these photos, you do have thin stretchy skin with lots of stretch marks. I believe you would do best with two surgeries. A tummy tuck and a breast lift create vector forces that pull opposite each other. You already proved that your tissues are lax and it is difficult to get a good inframammary crease for you. If you were my patient, I would do the abdominoplasty first. The breast lift (no implants, your tissues won't support them) should be done later, to raise the fold and an internal bra would give you more control and stability of the tissues in the long run. Patients frequently want to have one surgery, but if you choose that route, you will have further surgeries to address the above issues I mentioned. You could have both done at the same time with great results, in fact this is commonly done at the same time. Each surgeon has their own preference however on how they do things and how they get the best results possible. I would see multiple surgeons and get a consensus

Another option is the first procedure consists of the abdominoplasty and inframammary breast lift with removing the excess skin in a horizontal ellipse with the goal of lowering the nipple areolar complex.  Then would wait 6-12 month prior to considering a breast implant and possible internal mesh.

Your case is a difficult case and requires a virtual consultation. My first feeling is to do the breast surgery first because you need a inflammatory crease fold revision to correct the issue with the implant and might also use a internal bra for the surgery expect that fee to be $10,000-$15,000. Then after 3 to 6 months, a full tummy tuck possible any revision needed for the breast. Again best to virtual consultant with.
