14 months post op - how can I improe these results? (Photos)

Over one year post-op from a dual plane breast augmentation with 460cc high-profile implants. I was advised pre-operatively that I did not require a lift. My concerns are low implant positioning/bottoming out, low nipple position, asymmetry, wide spacing between the breasts, and lack of upper pole fullness despite implant size. This has not improved with time.
