Question

I have 465/475cc high profile saline want to downsize about 100cc and change to silicone. Ive always felt they are too big for my petitie 5”3 frame and find myself self conscious and hiding them. i also would love more cleavage. my natural breasts weren’t very close together either but more concerned about the projection and feeling out of proportion, I don’t want to get a lift. Have no children yet, one day I’d be ok with it but not now. .

breast revision surgery is the most difficult plastic surgery procedure performed. Botched breast surgery alters normal tissue planes and laxity, the ability of breasts to heal properly, and leaves scar tissue that dramatically affects the level of difficulty of the corrective surgery.
Many patients come in to correct poor outcomes from their initial surgeries. The most common problems are due to implant malposition, bottoming out of the implant, poor scarring, and incorrect implant size.
While the type of revision required will depend on the result of the previous surgery and the desired result of the patient, some revisions involve extensive pocket repair that may or may not need specialized external bras.
If you are considering breast surgery revision, you should speak with a board certified plastic surgeon and have yourself properly assessed. Only after a thorough examination you will get more information and recommendations. Also, you have to make sure that your surgeon understands your breast augmentation goals.

Thank you for your question. Downsizing by about 100 cc and switching to silicone can likely give you a softer, less protruding, more proportionate look. Silicone implants also tend to feel more natural than saline. However, achieving more cleavage becomes more difficult if your natural breasts are widely spaced, and reducing size can sometimes decrease cleavage unless the implant width, pocket position, and profile are carefully selected. If your nipples are still well-positioned, you might avoid a lift for now, but if there is any sagging, downsizing alone can sometimes make it more noticeable. Schedule an in-person examination with a board-certified surgeon to measure your breast width, evaluate skin and nipple position, and determine which silicone implant size and profile will give you a smaller, more balanced result without making you feel too far apart or deflated.

Based on your photos, you shouldn't need a lift. I would recommend deflating your saline implants 4-6 weeks before your scheduled surgery to allow the tissue to recoil and recovery. This will further decrease the risk of needing a breast lift when downsizing your implants.

I would recommend seeing an American Board of Plastic Surgery certified plastic surgeon, preferably one who has also completed an Aesthetic Society endorsed fellowship as well. I would percutaneously deflate your implants in the office 4 weeks prior to proceeding to the OR to remove the deflated saline shell followed by new implants with or without Galaflex for pocket support. The pocket may also need internal tightening.

You can downsize and switch to gel, the combination will give you better proportions and a more natural look.

You shouldn’t need a lift, but you may have some slight skin laxity and less over fullness of upper poles. Again, more natural appearance.

Based on your post photos, they downsizing by around 100 cc and switching to high profile silicone should give you a more natural appearing breast.
