I think the basic issue is inadequate implant support, which is the primary reason to use an internal bra. Options are Galaflex or other mesh materials like TIGR. More suturing is unlikely to help, since it doesn't reinforce the pocket.

Larger implants will provide more upper pole fullness. The aesthetic benefit depends on your goals. In general, as implants get larger, the breasts look less natural. As you haven't provided the size of your current implants, it is not possible to comment on the change in appearance with a 500cc...
