I am about to turn 43 and have some extra skin on my upper eyelid. I'm thinking about a bleph surgery but I've always had hooded eyes and I love them. I don't want to alter my natural eye shape (like Renee Zellweger did). I would like to have them more like they were 10 years ago! Very nervous to alter my eyes! Not the best pics to represent issue but only have a handful on my computer. Also not sure why they are sideways. Thank you!
Answer: Don't get sweet talked into a bad surgery. Saying the surgery is about technique without saying what technique should be used is a warning sign. When I look at your pictures, what I notice, is the position of the eyebrows. They are low and the medial eyebrow heads are relatively wide. It makes me wonder if you are getting Botox that is effecting your eyebrow position. If that is the case, I strongly recommend letting that service wear off before spending time getting eyelid surgery consultations. This might be 3 to 4 months after the last Botox service you have had. You might do beautifully with a very precise anchor blepharoplasty but it is also possible that you need a conservative, endoscopic forehead lift in addition to the eyelid surgery. There is no substitute for a detailed personal assessment.

It is absolutely possible to conservatively remove skin from your upper eyelids so they won't be overdone. You must relate to your plastic surgeon exactly what you are looking for and take photos of when you were younger. Also ask to see his/her pre and post operative photos, so you know you have someone you are comfortable with. Yes you can if properly done without removing fat and too much skin. often a brow lift in combination will keep your brows from being pulled down and corrects the subtle but meaningful brow ptosis that occurs with age and allows meaningful improvement with a conservative blepharoplasty.

You can maintain your natural eyelid shape with a very conservative SKIN only upper blepharoplasty. This means your surgeon needs to avoid removing any eyelid muscle or fat. You need to explain your expectations to your surgeon and together come up with the surgical plan. Communication between you an your surgeon is very important in maximizing chance for surgical success. You need to find a surgeon that listens to you and follows your aesthetic ideals, rather than his/hers. Look at before and afters and read the explanation for the photos.
