I am looking at 500, 550 or 580 CC for a large chest but not ridiculous on my body. Currently, an A cup, want to be about DD. (P

From your pictures and your description you appear to have only modest soft tissue coverage. Placing overly large implants will result in increased risk of a number of problems including contracture and rippling. A number of factors must be considered in determining the optimal implant size.Gion from an A to a DD is fairly aggressive in one operation. Discuss your desires with your surgeon to determine if this is reasonable for you.

Hello! Thank you for your question. In general, implant size does not correlate with bra cup size. The cup size itself will vary from manufacturer to manufacturer as well as who is doing the actual measurements. Thus, cup size is never a reliable indicator for your breast size. I typically encourage my patients not to communicate her desires in cup size but more on the actual look and appearance. Good communication between you and your surgeon of your expectations is warranted - choosing your surgeon wisely is the first step. Discussion of your wishes and having an honest and open dialog of your procedure is mandatory (e.g, implant type, incision, placement, lift, etc). I have found that photographs brought by the patient is helpful to get a visualization of the appearance you wish for in terms of size, shape, fullness, etc. In addition, your surgeon's pre and postoperative photographs should demonstrate a realistic goal for you. Once this has been accomplished, allow your surgeon to utilize his/her best medical judgment during the procedure to finesse the best possible result for you after preoperative biodimensional planning and fitting the right implant for your breast width. Hope me that this helps! Despite a good description of your goals, online recommendations will not be that helpful. The most important factor when sizing implants, is working closely with your surgeon in person to find the implant size that gives you the look that you desire. Careful measurements of your breast and chest wall will help determine which size implants will fit within your natural breast footprint. Taking into account your desired look, and your measurements, an implant is chosen that specifically fits your body. Unfortunately, online recommendations are not that helpful. Make sure that you have a good relationship with your plastic surgeon, so that you two can work together to find the right size. In my office, patients try on implants in a bra and clothing. There able to see how they look in the mirror, and in photographs. Women are also encouraged to bring in wish pictures, so that I can get a sense of the look that they would like to achieve. Thank you for your question. "Tissue based planning" relies on your chest and breast measurements and is the safest way to ensure good long term results. Your surgeon should determine what size and projection are necessary to give you your desired result based on your body's measurements. best advice you can receive is from an in-person consultation.  500-580cc is a large implant.  If this will safely fit in your breast pocket truly depends on your individual breast measurements.  The size,
profile, and shape of the implant is based on your desired breast
size/shape, your chest wall measurements, and soft tissue quality. 
This decision should be based on a detailed discussion with
equal input from both you and your surgeon.   Make sure you
specifically look at before and after pictures of real patients who have
had this surgery performed by your surgeon and evaluate their
results.   The most important aspect is to find a surgeon you are
comfortable with. I recommend that you seek consultation with a qualified
board-certified plastic surgeon who can evaluate you in person.

The best profile and volume implant to use depends on a woman's expectations coupled with her physical findings before she undergoes an augmentation.  In my experience a good place to start is looking at how wide the breasts are.  Picking implants that are about as wide as the breasts are will typically provide a nice look regardless of whether the implants are moderate profile, moderate-plus profile, or high profile.  The profile used will vary depending on how noticeably different the woman wants her result to be.  Higher profile implants create a more noticeable change.  However, it is possible that higher profile implants, over several years, will create more thinning of the tissues.  With more thinning of the tissues over time, the implants may become more noticeable, with ripples that may possibly be felt and/or seen.  Each woman will have a different result with her breast implants.  500 cc implants in one woman will not provide the same look or result in another woman because the tissue characteristics and the shapes of the two women's breasts are going to be different before either one of them ever undergo surgery.  Because of this it is not possible to tell what bra size a woman will wear after breast augmentation.  I have found that women who are personal trainers typically end up wanting implants that are slightly larger than I use in most other women.  It is not common in my practice to use implants that are over 500 cc's but those volume implants may be appropriate in some situations.  It is natural to have questions about which sizes and profiles and shapes of implants to use, and it is common to turn to the internet for answers.  The best way to achieve the result a woman wants most, however, is visiting with a surgeon who has a lot of experience with cosmetic breast surgery.  Tell that surgeon what you want.  She or he should be able to create a great plan for you based on her/his evaluation of your tissues and your expectations. 

Dear Flyingfitness:   In terms of picking the right implants (size and profile/projection)
to achieve your goals, generally speaking, I consider two factors when
selecting implants for my patients:
1. Dimensional planning
and 2. Gel Implant Sizing system
Dimensional planning – The measurements of your chest wall are taken.
Also, the breast dimensions including the height, width, and current
dimensions of each breast form the basis of dimensional planning. Based
on these measurements, the implant size is recommended. This will give
you a unique breast implant that is suited for your body frame. However,
there are some limitations of what size we can recommend. For instance,
some implants may just be too big for a narrow chest wall.  Hence a
physical exam is really needed to give you any meaningful advice.  
Gel Implant Sizing system – During the preliminary breast implant
consultation, you will be provided with an option to “try on” a variety
of implant shapes and sizes. You can also visualize the possible
outcomes of your surgery which helps you to get that perfect size to
give you the shape that you longed for. This way your preferences are
known and you can then pick a range of implants that will “fit” just
right to give a soft natural  fuller look. Hope this helps.

Based on your height and weight, to go up one cup size would be approximately 150-175 cc. That being said, to go up 3+ cup sizes would be in the 450-525 range. If you haven't already done so, please see a board certified plastic surgeon to try out some implants and see what you like, as the best judge of implant size is you. You should be afforded the opportunity to take your time and try out implants more than once to make sure you're comfortable with your choice - in other words you shouldn't be given 5 minutes to make a decision you have to live with for many years.

Hi FlyingFitness. Congratulations on your decision to undergo breast augmentation! Your surgeon would be best able to make sizing recommendations as he/she has the opportunity to examine you. Generally, the difference in volume between one size up or down is not significant, but the implant dimensions of one implant over the other may match your body better. Additionally, your tissue quality, thickness and pliability may determine whether a larger implant is recommended for you or not. In general, larger implant sizes have a more profound negative effect on the breast tissues over time; based on your photo and description, these larger impant sizes may be more problematic. Additionally, implants (or larger breasts for that matter) can interfere with athletic activities. Best of luck to you, and I hope you are able to achieve the results you are looking for!

Thank you for the question. Despite your good description of body type and goals, online consultants will not be able to provide you with specific enough advice to be truly helpful. Ultimately, careful selection of plastic surgeon and careful communication of your goals (in my practice I prefer the use of goal pictures, direct examination/communication in front of a full-length mirror, in bra sizers, and computer imaging)  as well as careful measurements (dimensional planning) will be critical.
Generally speaking, the best online advice I can give to ladies who are considering breast augmentation surgery ( regarding breast implant size/profile selection) is:
1. Concentrate on choosing your plastic surgeon carefully. Concentrate on appropriate training, certification, and the ability of the plastic surgeon to achieve the results you are looking for. ***Ask to see lots of examples of his/her work.
2. Have a full discussion and communication regarding your desired goals with your plastic surgeon. This communication will be critical in determining breast implant size/type/profile will most likely help achieve your goals. 
In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. For example, 
I have found that the use of words such as “natural or ridiculous” or "DD cup” etc means different things to different people and therefore prove unhelpful.
Also, as you know, cup size varies depending on him who makes the bra; therefore, discussing desired cup size may also be inaccurate. Again, the use of computer imaging has been very helpful during the communication process, in our practice.
3. Once you feel you have communicated your goals clearly, allow your plastic surgeon to use his/her years of experience/judgment to choose the breast implant size/profile that will best meet your goals. Again, in my practice, this decision is usually made during surgery, after the use of temporary intraoperative sizers.
