The follicles that are transplanted will retain the properties of the donor site.  Thus, they will not die from DHT. However, understand testosterone therapy can kill more follicles that are DHT prone. Other common questions are answered in the attached video.

There is no hair length requirement for having a hair transplant surgery.  If you are having a FUE donor hair harvesting, your donor hair needs to be 2 to 3 mm in length.

And thus, won't be covered by insurance.  Any underlying depression must be treated by a psychologist or psychiatrist, not a plastic surgeon.  While I do agree hair loss is traumatizing and can make one feel depressed, anxious, and unnatractive, insurance companies simply don't care....
