I ask my patients what their top 3 goals are. I then cater a surgical plan to meet those goals. There unfortunately is a lot of marketing mixed in. Let me assure you that there are no easy or quick fixes for any problem. If you want long lasting results that do not require a lot of maintenance then surgery is an option. If you want to have mild to moderate results and are ok with more time commitment for multiple treatments (and often times long term more money), then you can proceed with a multitude of nonsurgical treatments. They all have various benefits. However, the above holds true regarding time commitment, cost, and degree of improvement. There are many surgical approaches to facial rejuvenation. They have a multitude of names that confuse everyone, especially when surgeons begin to name them after themselves for marketing purposes. To address neck laxity, bands, and fat excess a neck lift alone can be sufficient. A facelift will address howling and deep nasolabial folds. Fat grafting is a good tool to fill volume loss. Lasers or peels accomplish the same thing. They smooth out the skin like sand paper. This is also another adjunct in addition to skin care regimens. A board certified plastic surgeon, passionate about facial rejuvenation can have an educational discussion to create a plan catered to your needs/goals. Unfortunately this issue can only be addressed with another facelift to address the laxity of the issues and excess skin. I would highly recommend seeing a board eligible or board certified plastic surgeon for a physical exam and accurate analysis with safe recommendations for potential surgical options. Best of luck!
Those deepLines are difficult to deal with. Because of your skin type you might want to consider a direct excision of the skin ion that area. Google direct skin excision of nasolabial folds to see examples. It may sound like a weird thing to do but is a reasonable option in your case.
Unfortunately, you're going to require another face/necklift to revise the loose and inelastic skin your face and jowls.. The wrinkles on your upper lip can be smoothed out with either laser resurfacing or a dermabrasion done at the same time.
The main problem now is skin quality and lines. The pull is a bit too vertical for my taste but I would just do an aggressive full face laser at this point to improve the perioral and cheeks (and rest of face). I would use a Sciton TRL laser which is a full field Erbium laser. These are mostly done unde rlocal anesthesia but twilight or general anesthesia is available.
What can I do to revise cheeks and mouth after SMAS facelift? Only revision of upper facelift can improve the oral jowling appearances. Ask your original surgeon for a revision fee. r seek virtual consults. I charge $10,000.
Your skin has very severe sun damage and surgery can't fix that. This is why you have those telltale swooping lines in your cheeks. You might seek a full face laser resurfacing to remove some of this damaged skin, fillers to remaining lines, and then stick with excellent sun protection after.

I would not call those pixie ears at all. With pixie ears it looks like the earlobe is melting into the jawline and is tethered. There’s a small web at your left lobe. Should be easy to correct
