Female, born 6/19/98, 93 pounds, 4’11”, dry skin with acne I am interested in having a rhinoplasty. I am 50% Korean, 25% Chinese, and 25% white. My nose hooks down when I smile, the front view is wide, and curves from the side view (with and without smiling). The nose hooking down bothers me but I don’t want it pushed too high like a “pig” nose.

Answer: I can recommend a couple of steps for the nose you want. The tip of your nose shows drooping, which is presumed to be worse when smiling. The drooping can be corrected by nasal tip projection surgery, which is possible through a columellar strut graft or septal extension graft. Don't worry about becoming a piggy nose. It is possible to make a natural and pretty shape. However, one disadvantage is that the tip skin is thick and heavy, so it is necessary to use a sufficiently strong cartilage graft. To do this, it is necessary to compare which cartilage is stronger between ear cartilage and nasal septum cartilage, and depending on the condition, the use of rib cartilage may be necessary, but the possibility is not high. For the correction of bulbousness of the tip, debulking of the fat tissue and alar cartilage realignment will be required. Even if you didn't mention a wide nasal bony base, but osteotomy can be used to transform it into a slimmer and more defined nasal bridge if you're interested.
