How close can I get me to this? After a bunch of consultations I'm not sure, who can get me there? (Photos)

Straight to the point, if a doctor were to say I can do this for you, I'll come right now. anywhere in the US. After a bunch of rhino consultations, I am just not sure. I have a bulbous tip and large nostrils that flare even more when I smile. Whom should I speak to, in order to go from left picture to right?

Hello and thank you for your question and the pictures. Addressing the shape of the nose (particularly the nasal tip) is actually one of the most common reasons people seek out and undergo a rhinoplasty. An open septorhinoplasty can best address your nose, and is particularly effective at addressing the tip size, shape, and definition! We can definitely get you a lot closer based on the pictures!

This truly a surgery of millimeters- and it requires exceptional surgical skill to achieve natural-looking results! Because your nose is the DEFINING feature of your face- I would only recommend seeing a surgeon who specializes in rhinoplasty procedures, and who has significant training and experience performing these surgeries. I actually trained under one of the world leading rhinoplasty experts (who performs 1,000+ nosejobs annually).

I recommend for you to setup a formal consultation with me (or another plastic surgeon who is extremely comfortable with rhinoplasties) to thoroughly discuss your surgical goals, undergo a formal examination, evaluate your options, and develop a personalized plan just for you. I hope this helps!

Based on your images, the result seems reasonable. The one thing to remember is that no surgeon, no matter how good he or she is can guarantee a specific result. You didn't overly narrow your tip so that should be fine. Getting the nostrils to look a certain way may be more difficult. The wideness can definitely be reduced. The shape, being rounder like you have in the after photo, can be tougher. Be sure to see a rhinoplasty expert for consultation.

The photographs shown show two different types of skin. Your skin appears thicker than the ideal skin you desire. Skin thickness does not change the treatment plan. However, your tip can be improved. You have not provided a lateral view but you should be able to get the geometry similar to the desire photograph. Remember the skin thickness will not be the same.

Hello and thank you for your question. Based on your photograph, you may benefit from a tip refining rhinoplasty. Your surgeon can accomplish this by trimming, suturing, and reshaping the cartilage in your lower nose. Cartilage grafts may also be used to help improve tip refinement. I highly recommend that my patients focus much more on real longterm before and after photographs rather than 3D imaging.   I encounter so many patients in my practice who see me for revision rhinoplasty who previously had surgery elsewhere who feel like they were mislead into surgery by surgeons who relied heavily on 3D imaging without adequate real before and after pictures to back it up.  Photoshop is easy but real surgery is very different.

I always recommend that you carefully evaluate your surgeon’s online before and after gallery on their website for both quality and quantity of results. Make sure that there are hundreds of real before and after pictures which demonstrate long-term follow up results. If your surgeon is posting mostly on table results without real long-term follow up results, that is usually a major red flag.  If a surgeon is posting mostly just splint removal day videos without longterm follow-up pictures, that can also be a red flag.   Long-term results are much more meaningful than on table results or 1 week post-op results.  With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.
