Combination fat transfer/incisional gluteoplasty? (Photo)

Question

In 2019 I had a gluteal liposuction because I did not want to be able to see my gluteus from the front, through the space in my thighs, my butt was not large. The liposuction was concentrated towards the top of my gluteus, creating volume depletion, sagging skin, and worsening the situation. I prefer not to have an implant and am looking for a surgeon in California who would combine fat transfer with some incisional modification of the gluteal crease

In addition to my last answer to your similar query, mark my words, BBL will be a 100% failure in someone as lean as
 you. On the other hand, if you’d like to consider something such as a lower buttock tuck to remove loose skin/tissue and give you a better defined infragluteal fold then that is indeed an option.

Firstly, consider undergoing fat transfer to the lower half or third of your buttocks. Given your history of liposuction in that area, finding sufficient fat for transfer might be challenging, but potential donor sites could include the area over your superior iliac crest and abdomen. After this procedure, I suggest waiting 4 to 6 months to evaluate the results.
If you're still unsatisfied with the outcome after this period, a reevaluation would be in order. In this second stage, you might benefit from an infragluteal butt lift. This involves making an incision in the infragluteal fold (the crease between the buttocks and thighs), removing a crescent of skin and fat, and then pulling and tightening the buttock skin to enhance the shape and contour.
In summary, start with fat transfer and then reassess. If necessary, proceed with the infragluteal butt lift for further enhancement. Always consult with an experienced, board-certified plastic surgeon to ensure the best and safest approach for your needs.
