This is my 5th breast surgery. The first time I had double bubble. That a muscle release. Then trying to fix the muscle release a Dr placed a stratice graph for support on the lower cleavage area. He had to lower my creases and put a larger implant in. ,I got bottoming out. So he put permanent sutures, but they did not work. it’s been 12 years and now I will get them done again. The doctor wants to know if I want stratus graft or tigr martix. What is better. Any advice to prevent A reoccurrence

The question is not whether to use Tigr mesh or Strattice, but who is your revisionary surgeon. You have a very difficult revisionary breast surgery and need someone very experienced and this type of work. The use of meshes and foreign bodies will not help in your situation. I would recommend a smaller implant, capsulorrhaphy and a vertical breast lift. You should be able to have an excellent outcome.

Answer: Explant with Bellesoma Method At this time, I recommend explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Answer: Implants You need to go smaller since your tissue has not and is not supporting the weight of the implants. Mesh can only do so much, but cannot compete with heavy implants.
