I do have a strong strait nose which I do not wish to change. I am looking for balance in my face and profile. When I smile I look like I have high cheekbones because I tend to have a nice layer of fat around my face which makes me look quite young considering I'm 43. But over all the face is beginning to sag a little and new cheeks are what I am after. What placement would look best on a wide flat face with a roman nose? Cheers, C

As we age, we lose facial fat. The most visible area where this shows is the mid-face. Once high cheekbones are no longer balanced by full cheeks. From the photos posted, you have lost mid-face fullness. Fillers work great in this area to restore a full youthful cheek. As the filler is put in, the cheek bone can be augmented and "sculpted' with the filler as well to obtain your ideal shaped cheeks. This is much easier and faster than cheek implants, which is why I do far fewer than i used to do now that we have fillers that last a year or longer.

Facial fillers work well for this type of situation. Although several filler options are available, Radiesse (calcium based) works well in the malar area.

One of the most lasting and gratifying procedures for this problem is the endoscopic brow and midface lift. It is a hidden small scar procedure that restores the position of the lateral brow, cheek fat pad, and orbicularis muscle. Fat volume is restored in the pads beneath the muscle. Aestheically speaking, from the photos provided, your profile is fine as far as facial structure however there is excess fat and skin under the chin. From the front, the cheeks are quite flat and "jowls" are present indicating excess skin of the lower face. A minimal incision Facelift, shaping the cheeks with a Cheek Implant or a filler (I prefer Perlane), micro-liposuction of the neck and removal of excess skin through an incision under the chin would address your concerns.
