Do I need a revision for my high riding implants?

After 13 months, if they have settled high, a revision may be indicated to lower them. At times, changing the implant size/style may be also indicated.Options and expectations should be discussed with your plastic surgeon.

Your breasts look a little long from the front.  You have fullness up top on the lateral view but that could be related to the size and shape of the implant.  It is not unreasonable to ask for a revision to lower the impplant

It has been plenty of time for your implants to settle so by now they will need surgical correction and your surgeon should be very open to that discussion. Your breasts are out of balance… You have more fullness above the nipple then you do below and that is not the correct proportion. You may still be able to obtain that with the implants you have but your doctor should realize the correction that needs to be done…

Your implants can be dropped down surgically and the size of your implants reduced.  If the pocket is enlarged inferiority and laterally ( out to the side ) they won’t have as much fullness up top.

A breast revision with a smaller implant could be done at any time, at this point. The implant that was placed might be too large for you. Discuss this with your plastic surgeon, or find a new one.

Breast revision surgery is the most difficult plastic surgery procedure performed. Botched breast surgery alters normal tissue planes and laxity, the ability of breasts to heal properly, and leaves scar tissue that dramatically affects the level of difficulty of the corrective surgery.Many patients come in to correct poor outcomes from their initial surgeries. The most common problems are due to implant malposition, bottoming out of the implant, poor scarring, and incorrect implant size.While the type of revision required will depend on the result of the previous surgery and the desired result of the patient, some revisions involve extensive pocket repair that may or may not need specialized external bras.If you are considering breast surgery revision, you should speak with a board certified plastic surgeon and have yourself properly assessed. Only after a thorough examination you will get more information and recommendations. Also, you have to make sure that your surgeon understands your breast augmentation goals.

Thank you for your question. If you are unhappy with your result, you should first consult with the surgeon that did the initial breast augmentation. After that consultation you should find a few other board certified plastic surgeons for second opinions. I believe you would benefit from a breast revision surgery to give you a more natural look. The cost of surgery will vary on the surgical plan the board certified plastic surgeon gives you to correct the issues. 

Your concerns are understandable and, based on the timeframe since your surgery, I do not expect significant changes (in terms of breast implant position) to occur.Revisionary breast surgery will likely involve capsulotomy;  the procedure will likely involve release of the capsule of the lower breast, lowering the breast implants on your chest wall.  It is possible that your current breast implants will "do the job"; on the other hand, exchange to different implants may be necessary to achieve your desired outcome. Careful  preoperative communication of your desired outcome  will be important. My general thoughts regarding the potential need for revisionary breast surgery:  I advise patients to carefully consider their degree of concern with the outcome of the initial breast surgery and weigh these concerns against the potential risks/complications associated with the planned revisionary breast surgery.   For example, if the patient is concerned about her breast appearance on a daily or frequent basis, it is likely that revisionary surgery will be helpful when it comes to improving her quality of life.  On the other hand, if the patient's degree of concern seems minor and/or she is  infrequently concerned about the areas of concern, I would recommend against revisionary  breast surgery.Policy regarding coverage of expenses for revisionary surgery will vary from office to office and probably on a case-by-case basis as well. Costs of revisionary surgery may range from zero, to anesthesia/surgery center fees, to discounted surgeon's fees.
 
 Generally speaking, I suggest that patients communicate their concerns in a calm/constructive fashion. Avoidance of inflammatory terms (such as "botched") and/or "blame"  is helpful when it comes to maintaining respectful physician/patient relationship and communication.

Thank you for your question and photographs.  Overall there are several features of your breast augmentation that could be made to appear more natural.  Your breast volume appears overly full on the upper chest.  Your nipple is not positioned well over your implant.  The bottom of the breast is not as round as the upper pole.  Without seeing you in person, I could not be certain of what is causing the imbalance.  Reasons might include an implant position that is high, a nipple position that is too low or an implant that is too large for your skin envelope.  All of these are things can be addressed with a revision surgery.  A revision should be able to easily come closer to what your idealize.
