When I smile I have cheeks/skulls "coming out" but when I don't it's flat.

These fillers (Voluma, Bellafill) and Sculptra can also be used to create visible cheekbones. Implants or fat would be alternatives. It is best used in areas where there is thick skin that needs lifting (lateral cheeks, jawline, temples, etc). Make sure to seek out an expert injector who will take into account both the resting and active volume in your face. Best of luck.
