If the desired size/fullness is not achieved in one procedure, do I have to keep paying for fat injections?

Ensure that the specialist you are planning to see understands what you hope to achieve with this technique as well as what can be achieved with it. Lipoinjection is the process of shaping the body. The fat that is extracted from the body is then used to fill out other parts of the body, such as the breasts, in order to enhance their shape. In the case of the breast this technique is used to provide symmetry and improve the body contour and therefore does not increase more than one size. the price is entirely practice dependent. If it was planned to be staged than the cost may be included. If its a revision, typically surgeons only ask the patient to pay for the surgery center and anesthesia.

Each fat transfer has a cost for the surgeon's fee and the medical supplies and facility and anesthesia. Many times, multiple transfers are needed. Otherwise, implants may be the better choice.

Each surgery has expenses - supplies, medications, staff, anesthesia, MD time. So YES each surgery has a fee. Best to fully understand the fees per surgery sessions first.

Generally speaking patients should be expected to pay for each procedure unless the second procedure unless it is considered a revision or results turned out differently than what was anticipated by the surgeon. Each surgeon will have their own policy and level of comfort with doing revisions or touchup procedures. Doing a second round of fat transfer is in my opinion doing a second procedure and it is not a touchup procedure.
If patients could have a free second round of fat transfer by saying they were unhappy with the volume obtained than all patients getting fat transfer would be asking for second procedures.
Even under ideal circumstances the volume increase obtained with this procedure is subtle at best.
There are occasional very impressive pictures on the Internet and many of these represent early results which can look very impressive but do not represent what the final results look like.
Fat transfer can increase breast volume by about one half cup. Cup size is subjective so the best way to review this is to review each providers before and after pictures during consultations.
Ask to provider to confirm how far out from surgery the after pictures are taken and ask them to show you as many before and after pictures as they’re willing to especially have patience with similar body characteristics.
Patients who want a substantial volume increase should consider using implants. This is also true for patients who want fullness in the upper half of the breast or those who are wishing to get more breast projection.
Patients should discuss all aspects of what’s included and not included with the cost during the consultation. This includes both secondary surgeries and revisions if needed.
