I had rhinoplasty 12 years ago to correct a large nose with a hump. I was happy for a long time even though my nose was never perfect post surgery. However, I don't know if I'm imagining it but my nose seems to be collapsing and getting worse. Is it realistic to build up the nose with some grafts so it is stronger and more natural looking? I think it looks fake and overdone at the moment. How difficult is this to do ? I'm scared it will go wrong and I will end up looking even worse.

With or without surgery, our noses change overtime.  And if your first surgery wasn't performed with longevity, strength, and breathing in mind, it is highly likely that it is losing strength over time.  The result is twisting and alteration of the appearance, particularly as ligaments weaken and tissues thin.  Your best option is a revision or secondary rhinoplasty.  To truly understand what is needed, and to understand your risks, I would recommend an in-person consultation with a board certified facial plastic surgeon that is well-versed in revision rhinoplasty.

Hi there! I can see exactly what you’re referring to. Your nose has been overly reduced - meaning the structure that used to exist is now largely gone. We see this with “reductive” rhinoplasties which seek to make a large nose smaller. There are definitely options. A revision rhinoplasty with a structure approach would seek to build your nose to a more natural shape with good support. A lot of the pinching and narrowing you’re seeing is a result of a lack of structure mainly from cartilage. These can be very challenging surgeries, but certainly possible and rewarding. I would seek consultation with a facial plastic surgeon who’s very well versed in complicated, revision rhinoplasty. Hope this helps you!

A revision rhinoplasty can accomplish significant improvement in your nose, but it will not be perfect. The projection of your nose can be reduced, the nasal bones narrowed, and cartilage grafts will be required in the tip to prevent any further collapsing. It's important to know how much cartilage is left over on the inside of the nose for grafting purposes. It would also be helpful to obtain a copy of the operative report to find out what was accomplished in the first procedure. Choose your second surgeon wisely based on extensive experience, since this will be a very difficult procedure
