Prominent blue-green veins of the periocular region - above, below or lateral (temples) to the eyelids - are common. Sometimes these veins (which are usually 1mm to 3mm in width) protrude above the plane of the skin. These veins are consistently successfully removed by Sclerotherapy. Treatment by microphlebectomy may leave small scars at the incision sites. Treatment by any light source has a much lower rate of success. This includes Neodynium YAG (1064nm & 532nm) lasers along with other wavelengths and IPL (non-laser, intense pulsed light). Often, at the time of laser treatment, the vein may appear to be gone due to spasm (contraction) of the vessel. However, this is a temporary phenomenon and the vein will reappear intact. After Sclerotherapy, there is redness and swellling; sometimes this is accompanied by bruising, all of which can require (unusually) up to 2 weeks to resolve. Unlike treatment of veins on the lower extremities, there is rarely brown pigmentation along the course of the treated vein.David Green, M.D.Dermatologist Bethesda, MD
