Question

Recently the Actress Susan Sarandon Admitted to Having Plastic Surgery ProceduresI was surprised that Liposuction can be effective on a woman her age?I am the same age 66. I am at ideal weight, but have excess fat under my neck, especially when I put my head down! I would think that skin would sag after fat is removed. How can this be known in advance? I can afford the Lipo, but if it doesn't look good what would a neck lift cost? I am also afraid of feeling a gag like feeling after a neck lift!

A neck lift would be reasonable from your description and pictures, but a physical exam would be necessary to give an accurate answer in regard to your specific circumstance.  Find a plastic surgeon who performs hundreds of facial procedures each year. Chin/neck liposuction is indeed possible at the age of 66, but frankly most patients at this age also need a neck lift to snug up the skin and muscle as well as eliminate jowls. The only for sure way to tell if you need all this is to schedule a consult with a board certified plastic surgeon whom can hands on examine your chin/neck. Costs vary widely from office to office but good chemistry and confidence with your surgeon is usually worth the extra money. And finally, there is a tight feeling immediately after surgery because the tissues were actually tightened up. However this feeling fades quickly in most patients over a week or two but rarely is "gagging". Glad to help...

Despite what Susan Sarendon may have had done, cervical (neck) liposuction alone in a 66 year old is likely to yield very disappointing results. Based on the photos shown and typical examples I have seen in 66 year old patients in my practice, cervical obliquity is generally more related to platysmal (muscle) and skin laxity than fat excess. More typically, I would recommend a Facelift for a 66 year old woman, which allows for repositioning of the jowl/ jawline, midface, and contouring of the neck. Although many patients have a tight sensation in the neck for 6-12 weeks after the procedure, this invariably goes away with time. Price of a Facelift will vary with geographic location and the individual Surgeon. However, I would like you to consider three important points about Facelift when considering price: 1) A well performed facelift is likely to last 10 years. On the other hand, quickly performed and less expensive procedures have a shelf life of 6-18 months. 2) Quality is remembered long after price is forgotten. 3) Very good Surgeons are much less likely to be inexpensive than mediocre ones are to be affordable.

At 66 years of age, there are 3 issues that will need to be dealt with, these are significant excess skin, fat above and below the platysma muscle, and the muscle bands in the neck will need to be tightened to give a comprehensive rejuvenation. In addition, the jowls are listed with a facelift procedure. Liposuction alone is not a good idea due to the excess skin which will be come worse. I doubt if liposuction alone would be beneficial. At your age it has to be combined with a neck skin tightening procedure which can fall under a number of terminologies. Cost should not be that much more. Whatever the difference in cost I think it would be a waste of your money to do liposuction alone. There is ,however, the occasional exceptional result wherein skin retracts after neck liposuction.
