it is hard to tell for sure without an examination. Most surgeons would recommend you to wait until you stop lactating and then get surgery. If you are considering surgery, I would suggest you consult a board-certified plastic surgeon. Only after a thorough examination, you will get more information and recommendations.

Under no circumstance should you have fat transferred while you are lactating. The cannula can penetrate a duct and could have lactating in the soft tissue ending up with complications.

If you were my patient, I would ask you to wait until at least 6 months or more after ending breastfeeding. The first concern would be that the breast parenchyma and ducts can be damaged causing milk to leak and collect in your breast which could become infected leading to abscesses or mastitis. This could also alter you ability to breastfeed if that is what you desire. And finally, we can't predict how breastfeeding will affect every breast, some worse than others. It is good to give it time to settle and deflate as much as it is going to before altering the breast.

No, no fat transfer should be done while you are lactating and you should avoid breast surgery until you have stopped breast feeding for 6 months to let your breasts completely settle down.

Thank you for the question. Obviously, if you are going to anesthesia, you are going to have to Pump and Dump. Putting fat into your breasts while breast feeding is probably a great idea as far as fat survival rate due to the increased blood suppl;y at that time. But! I bet you will have a hard time convincing most surgeons to do that for you while you're breast feeding.
