Dr Ting is a very competent, discreet and calm physician. She us also a talented "artist" when it comes to esthetics. She will do her best to accommodate your wishes but you should know how to express what your desires are. Her staff is very kind and Aspen outpatient center is very good.

I had Breast Lift several months ago. I'm ok with the results. My nipples continue to look strange and a bit uneven but other than that breast lift ok. I've been disappointed in the aftercare. It seemed difficult to actually see Dr. Ting but instead I saw the lovely ladies that work in the office. To date I've never been scheduled for the 3rd month exam.

The answer to this question is that you will first want the old mastopexy operative note to allow your surgeon to find out what surgery was performed previously and where the remaining blood supply to your nipple/areolar complex lies. Once the surgeon knows where the blood supply to the nipple is coming from, she/he will be able to safely place an implant without reducing the blood supply further. A transaxillary breast augmentation may be safest but that would of course use an armpit incision. An argument to use the old scars is that you may want your surgeon to revise the lower pole as the nipple position is on the high side and placement of the implant may stretch the lower pole further, lengthening the nipple to inframammary fold distance.
Individuals who perform a lot of pectoralis muscle workouts (bodybuilders, etc.) may have less discomfort and distortion if their implants are placed above the muscle (prepectoral or subglandular placement). When sizable implants are placed under the muscle it places significant strain on the pectoralis which will be aggravated with excessive repetitive pectoralis contraction. If the implants are placed dual plane where the origin of the pectoralis muscle is cut to create a pocket for the implant, the remaining muscle fibers are mostly those between the breastbone and shoulder. Repetitive contraction of this less effective pectoralis muscle can contribute to soreness and lateral displacement of the implants with time. If your work involves a lot of chest press type maneuvers you may want to consider subglandular implants.
In my experience fat transfer to breasts can cause some sensitivity after surgery that can last a few weeks. Most of the discomfort is from the liposuctioning, which is worse with power-assisted or vaser liposuctioning than traditional suction-assisted lipectomy, and even more uncomfortable if you also treat the liposuctioned area with radiofrequency, such as Renuvion.
I think the two issues are unrelated as long as you are not harvesting fat anywhere you had CoolSculpting. Freezing fat results in damage and inflammation to the treated area and subsequent scar tissue where the treatment was performed, making fat harvest from these areas unreliable and unpredictable in terms of viability.
Is there any bruising, implant displacement or malposition, or swelling? Tears in the capsular scar tissue that form around implants from external pressure can cause mild bleeding or pain as the tissue pulls on nerves and muscle fibers. New implants are less likely to be damaged by pressure but it is still possible if you had calcified capsules around the implants that can push into aging implant shells with enough pressure. This often happens in much older implants undergoing mammographic plate compression.To find out whether your implants were ruptured you can have an MRI without contrast to check implant integrity. If there are no external signs of trauma, you may want to try some tylenol or ibuprofen and rest (and find a gentler masseuse). If you notice any increasing firmness of the implant you should get imaging and if no rupture is present maybe consider Accolate to prevent capsular contracture.
