Pre-gyno surgery, I did not have any wide indentations on my chest when I flexed on my right side. I also did not have any holes or indentation on my chest when I lifted my arms out to the side or above my head. Now when I flex my chest, my right side caves in all outside of the areola and has an indentation line that runs all the way to the center of the chest. Left side has a hole/indentation that is most noticeable when my arm is lift to side or above head. Surgeon did not seem to notice

Answer: Indentations following gynecomastia surgery Indentations may form when too much tissue is removed or is removed irregularly. Looking at your photos, you did not have a significant amount of gland tissue before and likely too much was removed causing indentations. It is possible that your result may be early and with time it will even out due to swelling, however.

Answer: Can anything be done to correct this? Hello @gynoguy7012, thank you for your question. Since you had a different glandular process between one breast and the other, so the result will not be identical. After a reasonable time has passed, approximately one year after the surgery to see if it is possible to place some fat so that the difference between one tissue and the other is not so noticeable. The best course of action is to consult one or several board certified plastic surgeon for more information and options tailored specifically to your needs. Best wishes! Alan Gonzalez MD FACS

Answer: Gynecomastia surgery results Dear gynoguy7012, I understand your concern. However, without a proper assessment, it would be difficult to determine what went wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice. Answer: Concerns about gynecomastia treatment With your arms stretched behind your head you get an excellent review of what the subcutaneous tissues really look like. It’s a first hand reflection of how the liposuction and or glandular tissue was performed. You can tell yourself how much change the appearance has with your arm in a down position as the skin drops and creates a slight bit of skin laxity at the bottom of your chest. A perfect out, for gynecomastia should look just slightly over treated with the arm stretch behind the head or it will look a little under treated with the arm in a down position. This of course varies substantially from person to person. Young people who have never had to wait fluctuations should be able to be treated without overdoing it and get a high-quality outcome. Men over the age of 35 or anybody who has had weight fluctuations be on 20 pounds will have some skin laxity that would limit the outcome of the procedure. There are three variables that determine The chest shape in regards to candidacy for procedure and should be assessed. They are skin laxity, excess subcutaneous fat and breast glandular tissue. Each one of these can contribute to fullness on the chest and each is treated differently. Skin laxity is by far the hardest to treat without leaving scars. Excess fat requires quality liposuction, glandular tissue requires direct surgical excision. I’m curious if your surgeon used an energy device such as VASER? Revision surgery is more difficult than primary procedures and patients should be very careful about simply just signing up for having round two if they’re not happy with the outcome. if this is the best your surgeon could do with the primary procedure that it’s unlikely they’ll be able to do any better by going at it a second time. Collecting other peoples less than perfect work can be difficult when it comes to liposuction which is after all a permanent and irreversible procedure. To get quality assessments reliant in person consultations. During each consultation ask each surgeon to show you their entire collection of before and after pictures for gynecomastia surgery. Being shown a handful of sample pictures which most likely represent the best results of their entire career is insufficient to get a clear understanding of an average results look like in the hands of each provider. An experience provider should have no difficulty showing you at least 50 sets of before and after pictures of commonly performed procedures of which gynecomastia is I would say fairly common. perhaps with that procedure having 25 or $.30 before and after pictures is a reasonable number for someone earlier in their career. If you want to get a proper second opinion consultation then schedule those as in person consultations and avoid virtual consultations. Come prepared for a second opinion consultations by bringing with you all before and after pictures and copies of your operative report. These are all available from your surgeons office if you ask for them. They are part of your medical records which you have a right to. Good luck, Mats Hagstrom MD
