At 4 weeks, you should be able to wear whatever bra is comfortable without effecting your results. An underwire is fine.

Thanks for asking. I don't have my patients wear a bra with an underwire after most breast procedures including fat transfer for 4-6 weeks. A soft bra or other garment is used. There should be no issue with an underwire at 4 weeks post your procedure, but it's always good to go with your doctors recommendation for post operative management.
I would suggest asking your surgeon what he/she prefers. Your surgeon knows where the fat was placed and can best guide you.
When your breasts are augmented with autologous fat, the fat survives by acquiring new blood supply from the surrounding tissues in your breast. After 1 month, whatever fat has "taken" has clearly survived in its' new home and has adequate blood supply. It is highly unlikely that the underwire will effect the survivability of that fat, at that time. Therefore, it is my practice to maintain my patients in a sports bra for 4 weeks after surgery, after which they may wear whatever bra they are comfortable with. Thanks for your question!
