What can be done to improve nipple asymmetry? (Photos)

Question

First BA operation was back in 2014 (periareolar, under muscle, Mentor silicone 350 cc smooth round, moderate plus profile).Late this past year, I developed capsular contracture and extra-capsular rupture to my left breast (not sure which came first).  In March 2025, I had breast revision surgery and implants replacements (periariolar, 515 ccs silimed round micro textured surface, maximum profile).  Now, that I'm almost completely settled, I'm concerned about the asymmetry of the nipples, how my left breast appears larger and the limpness of my left nipple. I spoke to the doctor who performed the revision and he basically said it was the best that could be done, there's nothing visibly concerning and to "just go live my life" :( So now I'm seeking other expert's opinions to see what might have happened (normal or not) and if anything can be done to resolve this so I have a more symmetrical appearance.  Any help is much appreciated!

I understand your concern. However, without a proper assessment, it would be difficult to determine what is wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice.

You have reasons of concernon your left side the implant is too high, seems trapped by a over inclusion underneath the pectoralis muscle (eventually, does the implant shift upwards when you contract the muscle? if yes, that's it).

I would recommend waiting for more healing. I would also advise strongly against any more revisionary surgeries through the incision of the nipple-areola complex (NAC); it will create bizarre tethering around the NAC and increase your chances of capsular contracture.

Thank you for the question and the series of photos, but the photos are confusing if the last ones are the appearance as of now after the revision then there is something that is not right. I would request a virtual consultation to obtain more details but appears the revised breast is either too large of an implant or the pocket was made too large or could it be ruptured again?. As for the nipple complex that can be revised to look more symmetrical as far as shape, but for location, it might be difficult to move it more centrally. Again, best of virtual consult with. Expect any kind of additional revision fees to range from $8000-$12,000. Regards.
