Hello, hope you are well. The photo you provided shows excessive skin redundancy, especially at the cervicomental angle. Unfortunately, the less invasive options that were proposed (microneedling, etc) would not realistically be expected to make these larger rhytids go away. Without knowing when you had your primary surgery, the solution that makes the most sense based on the photo alone is to redrape and remove the excessive skin with a revision neck lift. It would be helpful to see additional views (eg profile), and discuss in person.
