Your BMI of around 45 requires that any body contouring procedure be performed in a Hospital outpatient facility with observation for that first 24 hours.Provided you will receive Medical clearance from a qualified Family doctor or Internist and understand the risks of the procedure-- you should be a candidate pending your in person consultation  and exam by an experienced Board Certified PS

First you would need to lose weight in order to be a good surgical candidate you would also need a Preop medical clearance. There are different types of tummy tucks. I would recommend a full tummy tuck. I invented a tummy tuck wear my patients wake up no pain. Recovery is much quicker then traditional techniques. I do not recommend getting a BBL and a tummy tuck at the same time. I like the patient to sleep on there back after having a tummy tuck. I like have patients sleep on there tummy after BBL. I share your concerns about how much your belly hangs and the problems it causes you. However being over weight is the single largest risk factor for poor healing as well as other complications of surgery. The fact you have already had a blood clot puts you at high risk of developing another. These clots can break off and travel to your heart and lung with a potential fatal out come. I believe the best advice  can give you is to talk to a bariatric surgeon. I believe surgical weight loss is your best bet for long term success.  Liposuction and other cosmetic surgery are meant for body shaping and for those near normal weight
  if you are tired if diet and exercise and not able to reduce weight .... ideal candidate for bariatric surgery.

It appears as if you're ready to make some improvements in your appearance and health. That is a great mindset. However, a tummy tuck,  liposuction, and BBL are not the appropriate first step in your transition. It really will be necessary for you to lose some significant weight before any elective type of surgery can proceed. Your BMI is currently 45. That is way too high and increases your risk of blood clots, which is already in your history, and death and a myriad of other complications. You will be hard-pressed to find a primary care doctor who will clear you for surgery for ELECTIVE purposes. You may be eligible for a weight-loss program, whether surgical or nonsurgical, through your insurance. Please, do not allow any surgeon to operate on you in your current condition for cosmetic purposes. This type of surgeon does not have your best interest at heart and is looking only to put profits before patients. You only get one life and your four children only get one mom. Lose weight first. Get to a reasonable weight for your height, and then enjoy the fruits of your labor. Your plastic surgery results will be much better and you'll experience a much more noticeable difference once you're at a reasonable weight. Best of luck. 

First of all get your diabetes status checked as rashes are the first sign,with increasing fat deposit you are liable to get diabetes and metabolic syndrome,count your calories and go for a brisk walk of atleast 5 kms a day with varying speeds.all the best

You are NOT a safe surgical candidate with BMI almost 45!!! Maybe seek a gastric bypass to lose weight.. But as for cosmetic surgery plus your previous blood clot history you need to rethink having elective surgery...

     The recommended BMI is less than 30. This is the recommendation of the American society of plastic surgery. The reason simply, is increased chance of complications. These include but limited to, pulmonary embolus, deep venous thrombosis, delayed wound healing and increase infection rate.
   Can I combine liposuction with another procedure? The short answer is yes but really limited. Firstly, we are only allowed to remove 1000cc or 1 liter when we combine procedures. The patient should know that when procedures are combined especially when it includes liposuction the complication rate goes up tremendously.

The appropriate procedure depends the amount and location of the excess skin. That's what your plastic surgeon will have to determine after the actual physical exam.
