Which procedure(s) can fix sanpaku big, round eyes? (Photo)

Question

I read that people got either infraorbital rim implants, lateral canthopexy, fat transplant, malar implants, or brow/forehead lift to raise their lower eyelids and make their eyes appear smaller. What procedure(s) would be recommended to help lift my lower eyelids, and make my eyes appear more almond shaped? Would the procedure(s) be a one-time surgery?

You have 2 issues contributing to the position of the lower eyelids. 1) lower eyelid retraction, 2) bulging eyes pushing the eyelid down. The main treatment you would benefit from is lower eyelid retraction surgery but additional bulging eye treatment may be useful as well. Consultation is necessary.

An oculoplastic surgeon who offers care for thyroid eye disease can perform this assessment.  I do not recommend orbital rim implants.  I think you need an orbital decompression.  You need to work with an oculoplastic surgeon who has a large volume of these surgeries.

Thank you for posting your photos. Not knowing your age or medical history, the first thing I would recommend is that you get evaluated to make sure you don't have any medical condition (e.g. Grave's Disease) that is causing proptosis which is just another word for "big, prominent, protruding eyes". Assuming all that is normal, then you should be evaluated by an oculoplastic surgeon as the procedures you would need in order to correct the position of your lower eyelids is a bit more complex. It is likely that you need a combination of treatments including: lateral canthopexy with spacer grafting and midface lift. These are all done at the same time through a lower blepharoplasty incision and they all aim to elevate the lower lid, support it, and re-create a more almond shape position. All these being said, you are still at a higher risk for developing lower lid malposition after surgery.The most important factor in your particular case, is to find a qualified surgeon that understands and performed these more complex procedures regularly. You have beautiful eyes and challenging anatomy. Lifting (NOT shortening) the lateral lower lid is your best option. You will hear different opinions regarding implants, but I would recommend surgery without implants unless you are displeased with your cheek appearance. Examination in person is important - see an Oculoplastics specialist to learn more about your options based on physcial examination of the eyelids and orbits. Best wishes.
