I had a revisional blepharoplasty that may have resulted in a hollowed eye. My research showed that Dr. Remonk was superb at fat grafting to fix doll eyes, sunken eyes, extra creases, etc caused from loss of orbital fat, however he is retired. He used the fat pearl grafting method. Is there any recommendations on the best doctor to fix my condition? Thank you so much for the honest and thoughtful feedback.
Answer: Don't believe everything you read on a website. Fat pearl grafts never really worked. They caused horrible scaring and eyelid irregularity. I have developed a method of lowering the upper eyelid crease and mobilizing localized eyelid fat to fill in the upper eyelid fold. Study the attached video. This approach is much more physiologic than stuffing it with fat grafts that will further scar the upper eyelid.

Answer: Seek an expert Upper eyelid hollowness is a dramatically underappreciated, yet incredibly important component of aging in the eyelids. With normal aging, our upper eyelid fat pads tend to retract and expose this upper lid hollowness, those with “deep set eyes“ are especially prone, and unfortunately old-school upper eyelid surgery that is overly subtractive can artificially create this hollowness early, actually looking worse than it did before surgery - the before and after photo in the link below resulted from overly subtractive upper eyelid surgery with another surgeon, and you can see the results after revolumization of the upper lids. Correcting this hollowness can happen with fat or filler injections into very specific planes of the upper eyelid, or transposing and rearranging the upper eyelid fat pads to provide fullness, as you see in this surgeon’s view video, borrowing the excess fat to move into the areas lacking fullness! To ensure you are receiving the highest level of care, seek out a modernly trained, new-school dermatologic surgeon, oculoplastic surgeon, facial plastic surgeon or plastic surgeon who is board certified and fellowship trained in one of these “core four” cosmetic specialties. In general, filler injection is more predictable, especially when there has been prior surgery with poorer blood supply. See following link and video. See an oculoplastic specialist.

Answer: Recommending a doctor for fat grafting of upper eyelids I'm happy you asked that question! Over the past four years,we have been doing stem- cell/micro fat grafting in the hollow upper eyelid periorbital area with significant improvement. The hollowness is softened and filled which creates a more youthful and attractive eye.This is exciting because until recently, we really didn't have reliable solution for this problem:)

Answer: Hollow Eyes and Fat Grafting This is a good question. Fat Transfer is a popular procedure now and many physicians are performing it. The eye area is a specialty area and can be fraught with complications, therefore, your surgeon needs to be experienced. I like to put filler and fat transfer in this area because this is one of the 1st places to show age as it hollows out. I call it the tired eye look or syndrome. It is the 1st area that makes us look older. The eyelid cheek junction needs to be smooth and this is the only way to achieve this. The upper eyelids as well can be filled as well as the temples. Because this is such a sensitive area, it must be treated accurately. The upper eyelids can be filled with either fat or filler. The ocular / orbital area needs to be treated as a whole unit and not as individual parts. This is important and the eyes are a center of communication. Therefore, it needs to be treated delicately. Good luck. I always say, your face is the diamond, polish it brilliantly and you will shine.
