I am 25 and had breast implants (415cc round high profile) a year ago but they dropped a lot more than I would have liked. I feel like my breasts lack any fullness in the upper pole and they are very full on the bottom and on the sides. This has also begun to create an ugly cleavage line like it is begging tugged down into an inverse U shape. What caused these things to happen and how should I address them? Why did they become teardrop shaped when I opted for round implants? Do I need a lift?
Answer: Implants You selected implants that are too heavy for your tissue and they have sagged. For better fullness, you will need to have them lifted and go down in size that your tissue will support. Going with larger implants will make the sagging worse,.

Answer: Explant and lift for droopy implants Your breast implants are too heavy for your skin envelope. 415 cc implants weigh about ½ kg each. The technique I recommend is explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained, if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired. Best Wishes,Gary Horndeski, M.D.

Answer: Redo breasts augmentation with mini lift Dear Moderngirl, Thanks for posting your pictures. It would have been helpful to see your before pictures. From observing your pictures the following is noticed: 1. Very large areolas and left one is lower. Mini lift (donut, Benelli, circum areola) will give you smaller and more attractive areolas, as well as nipple height symmetry. 2. Upper pole flatness. The mini lift will tighten the skin and push the implants upward , creating more upper pole fullness. 3. Lateral displacement of implants. Lateral capsulorrhaphy (internal bra) will narrow the pocket and give you fuller upper pole and nicer cleavage. 4. Wide cleavage. Medial capsulotomy ( scar release) will give you a much nicer cleavage. The best way to do it is through the sub areola incision, that you will have anyway with the mini lift. 5. Bigger implants will also give you fuller upper pole. you can switch your implants to 475 cc. Since you right breast appears bigger, you can use 500 cc on the left side. Always, consult with experienced board certified plastic surgeons who operate in accredited surgery center for your safety. Most importantly, check the before and after pictures of redo breasts augmentation in the photo gallery, to make sure that they are numerous, consistent and attractive with nice cleavage, perky, symmetrical and natural looking. Best of luck, Dr Widder

Answer: Bottoming out Dear moderngirl, bottoming out occurs when the implant falls below the current breast crease or inframammary line. It's more likely to happen with inframammary incisions, which is why I try not to use the inframammary approach to any breast surgery. Bottoming out can also occur if an implant is too large for the breast pocket. That is why it is important to choose an implant size that's correct for your body. I perform numerous measurements in the office to determine optimal implant size for my patient's bodies. If you are concerned, please check it with your plastic surgeon for reassurance and to further discuss your options and possibility of revision.
