Is it normal for cheeks to still be bruised and quite swollen three weeks after a facial fat transfer? (Photos)

I am 62 years old. I had fat transfers under my eyes, around the brow, and in the jowl area. Horrific swelling and bruising immediately after surgery. Eyes almost swollen shut. I am three weeks post-op and my cheeks are still huge, round black bruises on both of my cheeks. When I smile cheeks feel very stiff and smile is distorted. I look nothing like myself - more like a chipmunk. Since I am still bruised, can I assume that I am also still quite swollen? I'm starting to panic.

Swelling after fat transfer is completely normal, and can last several weeks to a few months. Some fat gets absorbed by the body, so surgeons will typically over-inject to account for that. So don’t worry, you will be fine. Just follow the instructions from your plastic surgeon. Good luck!

Please don´t panic. Fat transfer can be quite bruising and cause swelling for several weeks, even up to three months. Although fat transfer has very good long term results in the vast majority of patients, it is definitely more invasive than hyaluronate fillers, hence is done less often, due to the longer down time. In addition, over the next few weeks you will have reduced swelling and some actual resorption of fat. I always advise patients who have had extensive facial fat transfer to wait three months before judging the final shape appearance and outcome. I do not know the amount of fat you had injected into the face, but more fat means more swelling. Complications do include cellulitis, necrosis, subsequent atrophy and prolonged swelling. The attached paper may help reassure you that in the longer time patients are satisfied with the outcome.Your photos are very helpful. However, you must discuss your feelings and your facial appearance with your own surgeon as he / she can be the only person to advise on how you are, by direct examination.

Yes, it is common for there to still be swelling and bruising 3 weeks post-op after a facial fat transfer in a 62 years old patient. In dealing with this age group, I advise patients that the swelling may last as long as two months, especially around the eyes where the soft tissue support and muscular contractions that typically help reduce swelling is minimum.
It looks like you’re swelling has subsided greatly since immediately post-op. Hopefully, you’re using Arnica, which will help mitigate the swelling and bruising.
