Can I just get a neck lift to fix saggy skin and turkey neck? Or is a facelift necessary as well? (photos)

Getting a neck lift by itself will often leave puckering which looks unnatural so whenever you have a bad sagging neck, you almost always need a lower facelift with the neck lift for best results. After looking at your photos and without an in person consultation I would probably recommend a lower face and necklift. If you only lift the neck you will not be addressing the jawline. Again, I think an in person consultation would be necessary. 

Generally speaking, to achieve the best results in a neck like yours, there needs to be some work done in the face as well. The reason for this is that you have such a great degree of redundant skin in the neck, particularly in the upper aspect of your neck, that you will tend to see bunching of the skin on either end of the incision, if an incision is simply made along the posterior neck and behind your ear. It is basically what we call a dog-ear, a redundancy of skin that can be removed not just by pulling posteriorly on the skin, but also by lifting the skin vertically into the cheek and excising it around the front part of the ear.  Many patients in my practice, no matter what the age, are looking for improvement in the neck rather than in the face. The work that can be done in the neck to remove fat and tighten muscle can be done through incisions in the neck both anteriorly and posteriorly. But for the skin to be removed, as in your case, incisions usually need to be made in the area in front of the ear for all of that excess skin to be smoothed out and tightened.

A neck lift will excise any excess skin, giving your neck a smoother and more youthful appearance. Greetings,Given that you have had previous liposuction, you likely have some excess skin that needs to be removed to give you the improvement that you are seeking.  Although this can be done directly to the neck, the scars would not be acceptable for someone like you who has youthful, smooth skin.  Likely a facelift, although a big step, is the only thing that will give you the improvement that you are seeking.  The good news is that you will be good to go for many years by taking action now.  Good luck.

The neck can get sagging skin for many reasons including a lot of sun exposure, weight gains or loss, pregnancy etc.  Aging of the neck is very common before aging of the face.  It's one of the things that can be very frustrating to patients of all ages especially the turkey neck in patient’s with a family history. The face and neck keep changing as you age. Some of the changes are at the skin level, some at the deeper levels like the fat, retaining ligaments or muscles. There are several nonsurgical and surgical options to treat each of these different areas. Some can be done together and it really depends how much improvement you want to see as well as what part really bothers you. It is very difficult to give you opinion only on the basis of these photos. In person evaluation, including evaluation of skin and platysma muscle laxity is needed to make decision whether neck lift (without and with skin excision) or lower face/neck lift would be a better solution. There is no need to delay face lift portion if needed; you are young and well done lower face/neck lift will serve you for the next 12 years. Good luck.
