My alarplasty turned out poorly. It looks like the nostrils are stuck to the skin with no rim. Is there any combination of surgical or non surgical approaches that can be used to create the appearance of a natural looking alar rim?

In addition to flared nostrils you have a hanging columella. Both are simple to correct at the same time by a limited rhinoplasty

The areas that are circled in the photos represent the posterior portion of your lower lateral tip cartilages. If this area is widening with smiling, it is possible to narrow this area with rhinoplasty surgery. I'm not sure if this answers your question, so consider meeting with several...

Looking at your photos seems to give the faint suggest of a subnasal scar, as one would have from a liplift procedure. If this is the case, that would explain any difference because of the amount of skin lifted into the nasal base. In any event, the difference between the two sides can be corrected.
