Usually excising just the alar base or floor will just bring the nostrils in towards the midline. However, if you excise some of the alar wall as it approaches its attachment to the floor, you actually change the axis of the nostril from up & out to more anterior-posterior. This has the cosmetic effect of reducing what I call a nostril "snarl" look!
Without a photo and what you have in mind it is difficult to answer your specific question. Through an alarplasty incision, alar rim grafts can be place which can lower the alar rim, change the shape from round to more linear, and strengthen the ala.
After an alar base reduction, it is possible to change the angle of the nostrils using the same incision but it might involve removing some more tissue or changing the angle of the incision itself. This can be tricky but possible.RegardsDr. J

This is common to some extent after rhinoplasty surgery and maybe related to swelling or some uneven healing due to scar tissue. This can be treated with non-surgical treatments before thinking about more surgery. Your surgeon should be able to give you advice on the best way forward.
Nasal aesthetics has both an objective aspect, based on "norms" set by "the Great Masters" and modeling agencies, advertisers, marketers and Social Medial including TV and Movies, and a subjective aspect, based on how you perceive yourself! In your particular instance, it's probably a...
