If the volume of the prosthesis is artificially supported by capsular contracture the native breast can fall over the prosthesis creating a snout like appearance. This appears to be what is happening in the photograph provided.
The problem is not with your implants, but rather that you need to have a breast lift to optimize the appearance of your breasts. You should consider having a vertical breast lift procedure if your current condition is something that you would like to improve.
It appears from your pictures that the implants are in good position but your natural breast tissue has started to sag over them which is natural. You need a breast lift to center your breast tissue if front of the implants. The implants look in good position, you just have too much natural "sagging".
There are several reasons for breasts to sag and the implants remain high on the chest. First you may have formed some degree of scarring around the implants. This will tend to keep the implants higher up on the chest. The degree of scar formation around the implants has a lot to do with the feel and the look of the breasts. If more scar is formed the implants do not move very much and the breast implant will feel firmer. With the added scar the implants do not fall at the same rate as the actual breast tissue. Second if the implants were placed on top of the muscle this can also happen. The breast is naturally attached to the fascia of the pectoralis muscle. There are naturally occurring supporting ligaments which hold the breast on the pectoralis fascia. When implants have been placed above the muscle often over time the upper pole tends to stretch leading to the snoopy nosed look of your breasts. In your case you will need a breast lift as well as a proper examination to determine if the implant needs to be changed or scar needs to be removed so the implants move more freely.
I would suggest having someone else take photos - it is difficult to tell much from the angle of these photos.
You're correct that you're natural breast is sagging over the breast implant. The proper solution to correct this problem is a breast lift to lift the breast higher so that it rests over the breast implant. Attempting to lower the position of the breast implant will risk bottoming out or double bubble deformity.
