Hello Elenak14: I have read your question and all the answers below carefully. You must be feeling somewhat confused just now. Although seemingly contradictory, I believe there is truth in all of the answers below, which arise from the authors’ own experiences. Platysmal bands can be relaxed using Botox, which results in a smoother neck with skin that is more tucked up against the underlying muscles. It is impossible to tell whether you have platysmal bands from your photograph, but if you want to try something at home, you can jutt your lower teeth out in front of your upper teeth and then say “Eeeeeee.” If your muscles pop out and feel firm, you have platysmal bands that should respond to Botox. If not, and they stay soft, you would normally want to consider one of several options. Those options include radio frequency or high frequency focused ultrasound energy applied to the surface of the skin, such as with Pellevé, ThermiSmooth, or Ultherapy, or the application of radio frequency energy under the surface of the skin, such as with a ThermiRF procedure called ThermiTight. In have seen impressive responses with Pellevé, ThermiSmooth and Ultherapy, although the full results are not apparent for 6 months to one year after treatment. Sublative RF delivers RF energy to the surface of the skin, and will not penetrate deeply enough to achieve the tightening you are looking for. In the ThermiTight procedure, which takes one to two hours on the neck, a blunt cannula is placed through a tiny opening in the skin and radio frequency energy is delivered under the skin and near the fascia while carefully monitoring the temperature of the skin’s surface and under the skin, using two different monitoring technologies. In a facelift, the fascia is drawn to the sides of the neck, whereas in ThermiTight, the fascia is tightened in all directions via a process called bulk heating, resulting in a very natural-looking tightening of the neck, and also a smoothing of the surface wrinkles. The results can begin to become apparent in two weeks and are very significant at three months, with some continued improvement over the following 6 to 12 months or even more. Considering your time frame, I do think you might consider Botox if you have bands, and ThermiTight if you have looseness. The looseness seen in your photo is only mild to moderate, so you may never need a neck lift, but there is no reason you would not be able to have a facelift later in life if you notice additional loosening due to continued aging (or another ThermiTight procedure). You also do not have time to recover from a necklift before your very important event, and your downtime as well as your expense will be much less significant than having a neck lift. As technologies advance, the trend in aesthetic medicine is toward noninvasive and miniinvasive procedures, and with good reason. But there will always be problems that only a scalpel can correct.
