I have had two consults for a droopy tip and a small hump on the bridge and one surgeon said it would take him 45 mins as a closed procedure and the other surgeon said 2.5 hours as an open procedure. I'm really conflicted on who to choose and want to get other doctor's opinions. I am wanting to lift the tip and smooth the hump on the bridge.

Answer: Closed approach In general, the closed approach is faster but more technically demanding. I've been doing rhinoplasty for around 30 years. Currently, I am doing about 95% of my procedures closed. It is not only a faster surgery but also less traumatic for the patient.

Even most experienced rhinoplasty surgeons would have preferential technique for most of rhinoplasty problems. In some cases, one can select technique based on specific patient needs. Open rhinoplasty does take slightly longer than closed (15-25 min) but it makes no difference in overall result. I've done thousands of rhinoplasties and could probably tear through the basic mechanics of the procedure in 45 minutes or less, but that's not the point! Rhinoplasty is an operation of millimeters, and being an operation on a 3 dimensional organ requires careful and exacting technique with evaluation and re-evaluation each step along the way. Additionally, one has to be mindful of the post-operative changes and settling that can take place and be assured that the required support and symmetry will have been accounted for. You should ask prospective surgeons to see their before and after results of patients who had similar issues as the ones you have. You should also feel confident that the surgeon knows exactly what you want to accomplish and that he can do it. Beware of relying on morphed computer images because many doctors are better artists than surgeons. Plus, you can do things on a computer that may not be possible to do on your nose!
