This is very very common and in most cases you need not worry. I usually have my patients keep the opening moist with abx ointment. Over time the scars heal quite well, and rarely need any revisions. If they do need a revision later, Im sure your surgeon would do it.

That looks like excess breast tissue not just a little dog ear. I think you need an extension of your wise pattern if you want it all gone you may need an upper body lift.
