Dog ear revision under local anesthesia is a very straightforward procedure in most cases and not much to get worried about. You will need to be able to withstand just a few needle pricks in order to deliver the local anesthetic. After about 1 minute, your skin should be completely numb and you will only feel pressure, but no pain. I am not sure why your surgeon is requiring you to get blood work, x-rays, EKG, and urine tests though. That seems to be overkill in my opinion. 

That looks like excess breast tissue not just a little dog ear. I think you need an extension of your wise pattern if you want it all gone you may need an upper body lift.

based on your photo it appears you have more than just a dog ear.  it appears you have excess skin and fat lateral to your breast which can be improved with additional treatment.

Based upon the posted photo you have more than a "dog ear"! You have some excess skin but there is also a good amount of fatty tissue. It could be revised under local anesthesia with out blood work up in my opinion.
