I can share with you how I discuss our approach to hair loss treatment for patients like yourself in our practice. A little background: I’m a Board-certified cosmetic surgeon and Fellowship-trained oculofacial plastic and reconstructive surgeon. I have been in practice in Manhattan and Long Island for over 20 years. I am also the founder of TrichoStem™ Hair Regeneration Centers, based on a system we developed with PRP and a material called extracellular matrix going back 7-8 years now, which evolved out of a treatment after hair transplant. This treatment uses wound healing materials that ultimately became a standalone system for both male and female pattern hair loss. We treat patients from all over the world, and we have a lot of experience in hair loss treatment. It is first important to distinguish the different modalities using PRP or platelet-rich plasma. Currently, thanks to our work which began 7-8 years ago when we were first using this material to help people with hair transplant, we were trying to help patients heal the donor area from the strip harvesting from transplant surgery, as well as to improve the survival of hair grafts. A nice side-effect of this treatment was that thinning hair became thicker. To make a long story short, I took it upon myself to try to use this treatment of PRP and extracellular matrix to help stimulate hair growth in a predictable way for men and women with hair loss. Since we pioneered this treatment, and overcame a lot of challenges along the way, we saw very impressive results. We kept on working, and saw how effective it was in a very wide range of patients. At this point, it has become a standard that any other treatment in my mind is going to be compared. Currently, a lot of doctors having finally recognized the value of PRP. I say finally because 7 or 8 years ago, and for years after that, a lot of my colleague were not only skeptical, but openly dismissive about using PRP in hair loss. Now it has become quite popular, also thanks to some publications in dermatology literature which recognize the value or PRP. I would differentiate that TrichoStem™ Hair Regeneration from PRP treatments alone. You asked about PRP numbers of sessions. As it was pointed out earlier, the early signs of using PRP and extracellular matrix occurred when a patient was seen a year after transplant surgery, which meant the effect was clearly working for over a year. We looked back at our patients since we follow their progress very closely every 3-6 months for the duration of multiple years, often beyond 5 years. We have patients where a single injection treatment session lasted and sustained growth for over 5 years. With time, we learned that not everybody gets that longevity, and from that came the knowledge used to develop a classification system. What I learned is we have to look at patients individually by their variables including gender, age, age of onset of hair loss, rate of progression, degree of progression, and other variables such as previous surgeries, hormonal issues, and other medical related issues. From that we not only have an algorithm for individual patients, but also have a classification system from where we developed individualized treatment plans. For a 40-year-old man like yourself, we would likely with our TrichoStem™ Hair Regeneration system do a single injection treatment session. We would then follow you at 3 months, 6 months, 9 months, and would most likely predict a plateauing effect somewhere between the 15 to 18 month point. Then upon re-evaluation, we would consider doing a second treatment called the booster treatment, based on the experience we’ve had with other patients. We don't always do a booster, but for a lot of our patients who have moderate to advanced hair loss we do a second treatment. After the second treatment, we often see sustainable improvement for several years. Again, it is about the individual, which is why we developed individualized treatment plans. This is in sharp distinction to the current standard among a lot of my colleagues who are mostly new to using PRP, and basically share the same basic strategy and formulations whether it’s PRP alone, or PRP with Acell. The injection systems are either a monthly treatment sessions, to once every 3-4 months. A lot of our patients who have come to us who after getting treatment elsewhere were challenged getting multiple scalp injections, not just a single injection, done monthly, which in my opinion is almost a set-up for failure. I realize this is the current state of knowledge of a lot of my colleagues, and what we have attained is much more advanced level of experience, so we can minimize frequency of treatment. We treat patients from all over the world, so it is unsustainable for someone from the other side of the world to come back every month or every 3 months. We’ve learned with a lot of experience the longevity of the treatment, with all that experience, the individual predictor of the outcome, so from that we developed a treatment plan. It’s important for you to meet with doctors, and ask them about their experience with your type of situation. Currently, a lot of dermatologists are saying PRP works 50% of the time. A lot of our patients who come to us after going elsewhere for PRP alone will say very often they weren’t sure about what the outcome will be. Of course, that comes from not having a lot of experience, and not really understanding this type of management. Hair loss is managed, and t has to be done with experience, so it is very important for you to find an experienced doctor who can give you a clear understanding of the strategy, and the outcomes expected. In medicine, there are no guarantees, but there are certain probabilities, so it’s important to get that sense when you have a consultation.
