Hi @ckmd89! Thank you for sending your photos. A reduction of the hanging columella and straightening of your nose can be achieved with a closed revision rhinoplasty. I recommend seeing an expert revision rhinoplasty specialist for an in person examination. There are several options for you that can be determined after examination. A rhinoplasty in my opinion is one of the most difficult procedures to perform. In my practice, I utilize piezosurgery. Piezosurgery is a more gentle technique that allows for greater precision. Less bruising and swelling is common with Piezosurgery. In a typical rhinoplasty, a hammer and chisel technique is performed leading to more inadvertent fractures. Best of Luck, Dr. Nima!

A closed rhinoplasty can accomplish reduction of the hanging columella and straightening the crooked nose. All of the incisions are placed completely on the inside of the nose. No external incision is required. Also important to know how much cartilage is leftover on the inside of your nose, so important to have a copy of the operative report would be helpful.
