Severe tethering of neck / neck band causing tight feeling. Will revision cause more internal scarring? What's the best option?

I had an uneventful neck lift 2 years ago and have tethering of the skin to myneck and neck band which is extremely tight and fat now under the chin. I am concerned with a neck revision if I will have more internal scaring and a worse outcome. can this be fixed under local and what would you recommend. I feel the tightness in the front and the band all the time. would a submentalplasty under local anesthesia help correct this or should I have the deep plane lower face and neck lift.
