Because in the photograph there is a suggestion that the bridge of the nose is relatively wide at the base, it seems that a formal rhinoplasty with reduction in the width of the bridge tip and alar base would be the coherent solution to avoid unexpected aesthetic imbalances.

The procedure in question is called an alarplasty. The wings that you are referring to are called nasal ala and they are made of thick fibrofatty tissue covered by skin on the inside and outside. Ala can be reduced or narrowed by removing a small wedge of skin inside the junction of the nostril where it attaches to the cheek and lip. If done properly the scars are hidden in the crease and are not visible. This can be a standalone procedure or can be combined with a full rhinoplasty. Alarpalsty alone can be performed under local anesthesia.
