What type of rhinoplasty can I have done to target widening bottom part of nose? (photo)

I have noticed my nose has widen over the past 3 years at the bottom. The wings seem wide and the bottom part of my nose seems in a way fatty. What type of rhinoplasty can I do to fix this issue?

Thank you for your question and photo. During a rhinoplasty procedure, a technique called altar base reduction is commonly performed to narrow wide nostrils. To remove tissue, incisions can be made at the crease of the nose so that they are not noticeable. Since rhinoplasty is one of the most complex of all plastic surgery procedures, the requirement of experience, skill, and artistry cannot be overstated in order to achieve an optimal outcome. I recommend scheduling an in-person consultation with an experienced, board-certified facial plastic surgeon, with expertise in rhinoplasty techniques. They will be able to discuss your concerns and aesthetic goals with you to determine the best approach.

You are describing alar flare, which can be addressed in a rhinoplasty procedure by doing an alar base reduction. As for the "fatty" part of the nose, a rhinoplasty can also refine your tip and potentially reduce the fattiness of the tip if that's necessary (it usually isn't). Finally, you may have thick skin on your nose, which can be helped with specific types of skincare before and after the rhinoplasty.

Since the nasal tip is moderately wide, if you proceed with and alar base reduction alone it is likely to make the tip appear wider. Vision seems to function with a significant component of comparison and changing one element creates the impression that the neighbouring ones have also changed.

Because in the photograph there is a suggestion that the bridge of the nose is relatively wide at the base, it seems that a formal rhinoplasty with reduction in the width of the bridge tip and alar base would be the coherent solution to avoid unexpected aesthetic imbalances.

Collecting a wide tip and nostril base: a nasal tip plasty will narrow and define the tip and ,if called for, a nostril base reduction or “alar plasty” can be done to narrow the width of the nostril base.

The procedure in question is called an alarplasty. The wings that you are referring to are called nasal ala and they are made of thick fibrofatty tissue covered by skin on the inside and outside. Ala can be reduced or narrowed by removing a small wedge of skin inside the junction of the nostril where it attaches to the cheek and lip. If done properly the scars are hidden in the crease and are not visible. This can be a standalone procedure or can be combined with a full rhinoplasty. Alarpalsty alone can be performed under local anesthesia.
